Description
The goal of this case study is for you to present a response to a clinical scenario in which you demonstrate appropriate assessment, management and discharge of an individual experiencing health alterations when access to healthcare is suboptimal or compromised. Present a response to a clinical scenario demonstrating appropriate assessment, management and discharge of an individual experiencing health alterations.
Product
Individual Case Study (1800 words). The case study assignment is an individual assessment item. You may work collaboratively with other students to understand concepts in this course, but your answers must be your individual research, interpretation and application of the materials.
Marking Criteria
• Utilisation of appropriate assessment frameworks to identify the pathophysiology of the condition in the case study.
• Application of evidence to explain the actions the registered nurse / registered midwife take to implement and evaluate two (2) interventions to care for the patient in the case study.
• Critical appraisal of evidence supporting nursing / midwifery practice applicable to the patient in the case study.
• Application of the ICN or ICM codes of ethics to the actions of the registered nurse / registered midwife in patient discharge.
• Encouragement of access to, and participation in healthcare using the Social Justice Framework to underpin the actions of the registered nurse / registered midwife in patient discharge.
• Academic writing evidences academic integrity in the application of the APA7 referencing style.

Remember, this will not look like progress notes from a patient chart, this is more structured and uses academic language.
2)What assessment frameworks should I use in Task 3?
You should be using the secondary assessment, the organ system framework.
You should also you elements of the holistic assessment framework
3) What should I be writing in the assessment section?
Imagine that the person is sitting in bed in their hospital room and the RN is there assessing the patient.
You need to document what the nurse does in the secondary assessment, and the findings that they identify.
Cover multiple organ systems in this assessment, in addition to relevant social issues.
Allow 400 words for this section.
What we don’t need is a detailed procedure on how to conduct a secondary assessment. Instead, apply that to the patient, and tell us what is found.
4)Can the case scenario information be extrapolated?
The information in the case scenario does not provide you with as much information as you will get from a assessing a real person.
Students may extrapolate with additional information (for instance investigation results or screening results) as long as it does not contradict the information presented in the scenario.
5)How should the pathophysiology section look?
Identify the causes of the abnormal findings.
Identify the cause of the problems that will be addressed by the intervention.

What interventions can be discussed for the patient in the case scenario?
6) What interventions can be discussed for the patient in the case scenario?
Students can discuss two (2) interventions which correct the pathophysiology, these are considered essential.
Students can also discuss other interventions that are considered essential, to prevent the patient from harm while in hospital. Clearly identify how it is necessary for the patient’s care.
Nursing actions to monitor patients is not considered to be an interventions.
Present the nursing actions to implement the intervention.
Present the nursing actions to evaluate the intervention.
Don’t present an essay about the intervention.
Don’t present a plan.
Include references that show that your intervention helps fix the problem.

7)How should the Discharge Plan Look?
Describe nursing actions to improve the person’s access to health care or participation in healthcare.
These actions could address lifestyle factors, psychosocial issues or patterns of healthcare use.
Frame these nursing actions in at least one (1) of the elements of social justrice framework.
Identify elements of nursing codes and standards that also support these actions.
