COMMUNITY ASSESSMENT SURVEY PAPER
SOCIAL DETERMINANTS OF HEALTH & HEALTH DISPARITIES

DIRECTIONS:

1. Each virtual clinical group will choose a “community”.

2. This is an obstetrics and pediatric focus paper. The project must speak to how social determinants of health and health disparities affect the health and well-being of the maternal-child population.

3. As a group, do a survey of the “community” using the objectives of this experience and rubric as your guide.

4. The student will search the data bases of NYS DOH, NYC DOH And CDC websites for factors that contribute to health disparities and accessibility to healthcare, determine a cause and compile a report for submission during the final week of this course (see Rubric)

5. As a group, your report will be a written report of the survey to your instructor. Use the Learning Outcomes as your guide to structure your report (See rubric)
6. Discuss your experiences in searching specific neighborhoods and your findings.

OBJECTIVES:

1. Describe the “community” your group has chosen to survey in terms of its geographical parameters, demographics, socio-cultural composition.

2. Identify three major health problems that effect this community and impact the health and well-being of maternal child health and or the family.

3. Identify three factors found in the specific community that may contribute to illness during the childbearing/childrearing period. Give specific strategies that may help combat the negative factors identified.

4. Identify three health and human services agencies that are available within the chosen community to combat health and environmental problems.

5. Outline the steps you as a nurse would have to take to refer a patient/family to one of the identified agencies.

6. Explain the importance of a healthy community as it relates to successful childbearing/childrearing.
