Assignment guidelines
First formative assessment:
Students will watch a scenario on case conferences on week 6 of the blackboard site. Each student will choose one of the professionals and explore their role in relation to case conferences. This will be written up as a 500-word summary and presented to the group in week 7.
Each student should speak for a maximum of three minutes. Please give at least one reference. This information will form the basis of the 2,500 word first summative assignment.
First summative assessment: essay on case conferences
Students will submit a 2,500-word essay related to the case conference scenario. This will include the role of the professional they discussed in the formative assessment (the notes should be included as an appendix). The essay will look at the interventions and decisions made by the case conference, including relevant legal frameworks. The students should also discuss the impact these discussions could have on the patient and their family.

Essay Plan:
Introduction: (approx. 400 words)
• What is this assignment going to cover (this is what is given in the assignment brief above)?
• Describe a case conference
• Discuss the goals of a case conference
• Give a brief outline of your scenario
Legal frameworks which inform the decisions made by the case conference: (approx. 600 words)
• Describe the legal framework which underpin the decisions made in relation to your scenario
• Discuss how these legal frameworks were applied
Role of the Multidisciplinary Team: (approx. 600 words)
• Critically appraise and evaluate the effectiveness of the case conferences
• Discuss the role of MDT in relation to case conferences
• Discuss barrier to communication between members of the MDT
Impact of the decision of the case conference on the individual and their family: (approx. 700 words)
• Discuss how the individual might be given information about the process of the case conference
• Discus how they might be supported throughout the process
• Describe how they might feel about the process and the impact this might have on their relationship with the health care professionals
Conclusion (summary of the key points raised across the essay): (approx. 200 words)
• The conclusion should not introduce any new ideas but highlight the key issues and summarise the main points you identified in the essay.

Second Formative assessment: development of a care plan
In groups, students will develop a detailed care plan for a patient who has co- morbidities. Each group will decide what condition the patient has. For example, the group could choose to write about someone with type two diabetes, prostate cancer etc. Students will decide among themselves who will be responsible for writing each section (psychological needs, social needs, medicines management, risk management and nursing/medical needs). A template can be found in the assessment section if you chose to use it.
Students will work in groups five. The group members will remain the same for the second and third summative assignment.
Second summative assessment: group presentation
Students should present the care plan they developed for the formative assignment. All students are expected to present, delivering the section of the care plan they developed. The group should ensure that information is not duplicated in each presentation. Each student should present for a maximum of 10 minutes. The presentation will be followed by a debrief with the lecturer. Students are expected to justify the decisions made in the care plan, using references and guidelines. The presentations will be recorded for moderation purposes.

Third summative assessment: development of a resource
Each group will develop a detailed handout on the condition they presented in the care plan and presentation. The resource is NOT a care plan. The target audience for the resource will be first year student nurses. This handout should discuss the bio-psychosocial needs of an individual with the condition, including medical management and risk assessment. Each student should concentrate on one aspect (nursing/medical needs, psychological needs, social needs, medicines management or risk assessment), ensuring that the learning outcomes are addressed. The individual sections will be collated into a handout. Groups need to work together to ensure that there is no duplication in the resource.
Groups can use the template to develop the resource. Each section should be between 800-1,000 words.
