Clinical Medicaiton Review

You are a pharmacist working in a GP practice, providing a clinical medication review service.
The GP has referred Mrs to you for a full clinical medication review after she had a fall yesterday. She has not injured herself and the GP has ordered bloods and other relevant tests.
Part 1 Produce a plan for the clinical medication review with Mrs, using the GP case notes (file on BlackBoard).

Appraise the patient’s current treatment, identify any clinical issues, evaluate possible treatment options and produce a person-centred plan.
You will need to include a clear plan for managing each clinical issue, that you will discuss and agree with the GP (prescriber) after the consultation with the patient.

Presentation:

Part 1 word count – minimum of 1000 words and a maximum of 2000 words. You are NOT permitted any additional words, there is no 10% leeway on the word count.
You can include diagrams and tables, but tables will be included in the word count.
Use of bullet points is acceptable.
Your submission should be fully referenced (excluded from word count). Submit via Turnitin.

Suggested structure:

Consider utilising the following approach to appraise your patient’s treatment:
First, compile a comprehensive clinical problem / issue list for your patient.
Identify the current clinical issue that the patient is presenting with, analyse any significant test results, and then consider each medical condition in turn.

Include any other potential clinical and social issues you have identified.
For all problems / issues you have identified for your patient, assign a priority (high, medium or low) to the issue. Ensure you address the high priority issues in more detail than the medium and low priority issues.

For each clinical problem / issue:

• Appraise the patient’s current condition.
• Use appropriate guidelines and evidence to evaluate current treatment, place in therapy
and alternative / further treatment options.
• Include non-pharmacological, self-care and no treatment if appropriate.

• Consider the risk vs benefit of all treatment options, taking into consideration how this may
affect other aspects of the patient’s care.
• Provide a detailed plan to manage each issue, with decisions clearly justified.
• Describe monitoring requirements (for each medication and condition) and why these are
necessary.

• Outline the key counselling points to discuss with the patient.
Throughout the assignment discuss how you will ensure a person-centred approach.
