History taking in paediatric nursing is an essential skill that can take some time to develop. More often than not, in paediatrics you will be required to take a history from a parent or other primary caregiver.

The following task is designed to develop your skills in identifying key information from an interview with a parent and synthesising that information to create a health history for the child.

Scenario: You are a student nurse in a paediatric setting, observing a senior nurse, take a patient health history via a telehealth appointment.

Your role is to hand over what was said and observed, in the interview, using the ISBAR framework (see reading list for link to article on ISBAR, and paediatric health history. Follow the instructions below to complete the assignment.

Instructions

1. Watch this video of Sacha (nurse) conducting a history taking interview with Lisa (mother) and her child (Sunny).

2. Write an essay, that addresses the following (you can use these as heading prompts if you wish):

A. Document the most relevant information about Sunny’s health and health history from the video interview using the ISBAR format. That is, write a script for what you would hand over to the reviewing doctor. This information should include both the subjective information offered by the parent and child and also what you can observe from the video- objective health assessment

B. Discuss what information is missing from the interview and how this information would help to inform your plan of care. This discussion should be supported by current, relevant and academic quality citations.
