Assessment criteria
You will develop a field specific 2000-word essay based on a patient/ service user who you have cared for who requires long term care/ support.
You will critically evaluate nursing interventions of two care priorities identified through the assessment of your patient/ service user. You will critically evaluate and synthesize on-going care needs including multi-agency working. You will also be expected to discuss implications for your future practice.
You must include, as an appendix, a brief outline of the patient/ service user you chose to base this work on and key assessment findings. The key assessment findings should be structured using an appropriate framework.
You critically evaluate nursing interventions of TWO care priorities. The first care priority is DIABETICS and the second care priority is SCHIZOPRANIA. Focus more on the intervention by the multi-disciplinary team.
Remember it is not a reflective piece, but a paper that needs to be analytic and objective, based on current, best knowledge. The ‘Implication for Practice (10%) marking criterion may have elements of reflection and be written in first person if this helps.
Write this essay as a mental health student not still learning and observing all the interventions used by Multi-disciplinary team to give treatment, advice, recommendations. Most Importantly use the NICE guideline it covers recognising and managing psychosis and schizophrenia in adults. It aims to improve care through early recognition and treatment, and by focusing on long-term recovery. It also recommends checking for coexisting health problems and providing support for family members and carers.

ESSAY LAYOUT

2. Go straight to the point to talk about a patient on admission in the mental health ward you care for at placement who has two care priorities first care priority is physical health (type 2 diabetes) and the second care priority is Mental health (Schizophrenia).
3. Discuss the first care priority (Type 2 diabetes) and describe how you used ABCDE assessment tool to check the patient vital signs, blood glucose level, how the assessment lead to the diagnoses of type 2 diabetes. Talk about type 2 diabetes, the intervention advice, treatment, after care management given to the patient to prevent other crises in the future

Second care priority Mental health (Schizophrenia)
1. It is still on the same patient, describe the Mental health assessment tool you used to assess the patient and how you referred the patient to a psychiatrist nurse and doctor.

• promoting recovery and long-term care
• preventing and treating physical health problems and continuing to check for physical health problems
• support for carers
