This is a big project that involves a problem that has been identified in a hospital in San Antonio, TX. The hospital is not to be identified in the paper because my instructors would like it to remain confidential. The problem found is that the whiteboards in patient rooms are either not getting done entirely or are missing important information. I have already done the proposal part of the project. Please read the proposal as this is the change that this paper will be centered on. The remainder part of the paper will be added following the proposal section. After this paper is written it should be like a sort of manual that any other person in healthcare or in that hospital can use and understand the process on how to implement the proposed change (quality improvement). I’m including a sample paper that was done in the past for guidance only. Please do not copy the information but use it only to guide.. I know it is a similar topic but the approach to the change is different. This paper is aimed at quality improvement of patient whiteboard completion by ensuring better supply access such as using a magnet to stick on the marker and then to the whiteboard so that it won’t get misplaced, and installing a clip to place the cleaning cloth and providing teaching on why it is important to do patient whiteboards during morning and evening huddle (see proposal). I worked on a action plan table and Histogram that need to be included in the paper. I understand the action plan table takes up space in the document but please don’t use this towards the page amount I’m paying for as it was already done. The Gardners Task of leadership table should be counted towards the paper count for this order as it is not completed. The following are the parts that need to be included in the paper following the included proposal section:

Note:

• Must be in APA format (adherence to APA Manual 7th Edition) and include:
o Running head
o Reference list citations
o Tables & diagrams

• References must be within 5 years. Only Scholarly articles and do not use websites that are not reputable such as Wikipedia, and YouTube for example. These references must be properly used to

o Must include at LEAST 6 references!
o Justify the need for change
o Support recommended implementation steps
o Guide how to effectively lead the required change

Headings with Instructions:

• Background and Description of Problem

o Clearly define and provide a comprehensive description of the problem that is clear to the reader.

o Should consider elements such as the nature of the unit (it is a surgical floor), patient population served (Surgical patients pre and post-op, and recently a mixture of medical patients, older, young adults, bariatric..), the staff (mix of professional and Patient care technician personnel, demographics, etc.-see proposal to understand staff), and the type of health care organization in which the problem is occurring.

o Define the management problem needing change & identify a specific change model (FOCUS PDSA {discuss this model in the paper and how it will be used for the change process) and how it will be used to effect the needed change

• Causes

o the “who, what, when, where, why, and how” elements of the problem at the clinical agency – Must keep anonymity of agency.
o I performed a survey and made the results into a histogram.. see below “SURVEY” I think it would be good to maybe include that in this section. The nurses ranked what they thought was keeping them from completing the whiteboard.

• Evidence from Literature

o What does the evidence say to validate the problem? What are recommendations to address the problem? Think outside the box.
o Consider that some of your interventions will need to be aimed at motivating and leading change.
o Provide evidence related to how you will lead the required change.
o Scholarly articles no earlier than 2016

• Key Stakeholders

o Identify key stakeholders (who will be affected by the change, why they are stakeholders, how are they affected)

• Facilitators and Barriers

o Identify current and anticipated facilitators and barriers operating in the change situation to promote resolution of the problem.
o Connections between stakeholders, facilitators, and barriers should be explained.

• Outcome/Goal

o state SMART goals of the project with appropriate timeframe for completion.
o Goal(s) must specifically address the problem that was identified in the proposal. (Remember that goals are different than interventions!)

• Action Plan

o Describe the action plan for the management problem. You should utilize tables and diagrams to illustrate the following information, but they must be formatted according to APA guidelines. see Table 1 below and incorporate that into the paper. I worked on it to develop the plan on how things should evolve to make the change. feel free to change things around.. Please make sure to make sure tables are properly APA formatted. A succinct one paragraph summary should introduce the table.

o Tasks: (In table 1 below) list tasks necessary to achieve the project goal(s) in chronological order. Must include task(s) necessary to evaluate whether the goal(s) was achieved (i.e., measurement). Should reflect the problem solving (data collection, assessment of findings, planned interventions, evaluation) and change process (involvement of stakeholders)

o Resources Needed (items need to be in a table with a separate aligned row for each item):
 Budget must include a comprehensive total that incorporates estimates for:
• Materials
• Space
• Finance/Budget: Cost of staff time for planning. implementation, professional development, etc.
 Time Frame: approximate length of time to complete.
 Personnel: identify the person who will be responsible for completing each task
• Gardner’s Task of Leadership
