Introduction

In this week’s clinical experience, I encountered a complex case involving a 62-year-old male patient who presented with a range of cardiovascular symptoms. The patient’s chief complaints included shortness of breath, chest pain, and episodes of dizziness. As an advanced practice nurse, I undertook a comprehensive assessment to formulate an appropriate plan of care.

Assessment
Upon initial assessment, the patient exhibited signs and symptoms consistent with potential cardiovascular issues. He reported intermittent chest pain that radiated to his left arm, accompanied by shortness of breath during exertion. Additionally, he experienced occasional dizziness, particularly upon standing up quickly. His medical history revealed a family history of heart disease, hypertension, and elevated cholesterol levels.

Plan of Care
Given the patient’s symptoms and risk factors, my plan of care involved a thorough evaluation to identify potential differential diagnoses and develop a holistic management approach. The following three possible differential diagnoses were considered:

Coronary Artery Disease (CAD)
Rationale: The patient’s chest pain, radiating to the left arm, and shortness of breath are classic symptoms of CAD. Given his family history and risk factors, CAD is a strong possibility.

Hypertrophic Cardiomyopathy (HCM)
Rationale: HCM is characterized by an enlarged heart muscle, leading to symptoms like chest pain, dizziness, and breathlessness, especially during exertion. The patient’s family history of heart disease could predispose him to this condition.

Aortic Stenosis
Rationale: Aortic stenosis can cause chest pain and dizziness due to restricted blood flow through the aortic valve. The patient’s symptoms, especially dizziness upon standing, could be attributed to reduced cardiac output.

It is crucial to differentiate among these potential diagnoses to tailor the treatment plan accordingly. Diagnostic tests, such as electrocardiography (ECG), echocardiography, and stress testing, will be essential to confirm the accurate diagnosis. Collaboration with a cardiologist and other specialists is recommended to ensure the patient receives appropriate care.

Health Promotion Intervention

To promote the patient’s cardiovascular health, I emphasized the importance of lifestyle modifications. These include adopting a heart-healthy diet low in saturated fats and sodium, engaging in regular physical activity, managing stress, and adhering to prescribed medications, if necessary. I also stressed the significance of regular follow-up appointments to monitor progress and adjust the plan of care as needed.

This week’s clinical experience reinforced several important lessons for me as an advanced practice nurse.
